By filling out this Agreement we are able to:
- Share information with the below parties for your support. We cannot put in place support without your consent.
- Collect anonymised data about the number of disabled students studying here and what disabilities they have. This is for funding reasons and to plan our support.
You can opt-out of sharing information at any time. However, this will have an impact on the support we are able to provide and we will discuss any likely effect with you. Please contact us on 020 8417 7314 or by logging a query at studenthub.kingston.ac.uk to discuss opting out.
This form is also available in an electronic (Microsoft Word) or as a paper copy on request.

Why we share this information
Your academics & faculty student support team
To recommend reasonable adjustments for your course through a 'Summary of Support Needs'. You can access your Summary through the Online Student Information Service (OSIS).
The Exams Service
To inform the Exams Service what exam adjustments you’ll need.
The Wellbeing Service
To ensure that the Wellbeing Service at the University are aware of your mental health needs, in case you’re in need of their support.
University First Aiders including Security staff
To share a 'Care of' plan if your condition means you may need a First Aid response.
If necessary, to share concerns about your safety and wellbeing.
The LRC (library)
To put in place library adjustments for your course e.g. extended library loans.
Your Disabled Students Allowance (DSA) funding body
To receive your DSA funding documents from your funding body and to liaise with them to solve any DSA funding issues.
The DSA Needs Assessment Centre you go/went to
To receive your DSA Needs Assessment report from the Centre and liaise with them about your support.
Equality Enterprise & other DSA providers of mentors, tutors and student support workers
To send out your DSA funding documents to the provider, so they can set-up your disability tutor, mentor or support worker.
To liaise with them about your support.
If necessary, to share concerns about your safety and wellbeing.
Accommodation Service
To communicate information to them about your living needs.
If necessary, to share concerns about your safety and wellbeing.
